Creating a MyChart Account

To access your health records and manage your healthcare needs online, you need to create a MyChart account. Here’s how you can create your account:

  1. Go to the UW Health MyChart website at mychart.uwhealth.org.
  2. Click on the “Sign Up Now” button.
  3. Select “Sign Up Online” and follow the prompts to validate your identity. You may need to provide your activation code, which you can obtain by calling your clinic or health plan or by email or U.S. mail.
  4. Once your identity is validated, you will be prompted to create your MyChart username and password. Your username must be unique and at least six characters long.
  5. After creating your username and password, you will be asked to provide your email address and phone number. This information is used to send you notifications and reminders about your healthcare appointments and updates to your health records.
  6. Review and accept the terms and conditions of use, then click “Submit.”
